pdf-Office 5.3 (Mac OS X)

OpenOffice has better PDF form export capabilities, and it's free. BTW, NeoOffice has some minor bugs in PDF export, so (while NeoOffice is great for other purposes) I suggest the X11 version. [alert admin]
