You are right, but it's really not much of a disadvantage. In most cases it is better to delete the infected file and replace it with a new copy. Of course, this can be an inconvenience if you want to remove macros from MS Office documents and use them stripped. Otherwise, it's not a big deal.

With the Mac there is ClamXav Seventry that allows you to continusly scan files that have been changed in ceratin locations.

In addition to the ClamXAV Sentry mentioned by another poster, there is also a Contextual Menu Plugin that allows you to scan an item selected in the Finder by crtl-clicking (or right clicking if you have a mulit-button mouse.)

There is also a schedule function to autoscan at a specified time or day.
