make sure Cache is enabled per the Readme and then restart both pearLyrics and iTunes 5. that got it to work for me. awesome little program.

Hi, thanks for your feedback! There was an issue in the original V0.5 release being that if your main music library was called something else than "Library" it would not find the track to update. This is fixed now, so you should just download it again and give it another try (still version 0.5).
