The other thing that might be happening here is that the Agree / Disagree buttons are reversed. The Disagree button is defaulted to the Enter key, so if you are trying to get through the installation menu and hitting return to move to the next step, hitting return on Agree / Disagree will kick you back to the Safeboot warning screen. I had the same issue; turns out you can't boot into safe mode while you have an Open Firmware password enabled. (The Shift is ignored and it boots as usual.)Go to Apps/Utilities/Open Firmware Password and switch it off. Then, when you next shift-reboot, the login screen will display "Safe Mode" in red. On my older Mac I had to hold the Shift key for a good while; thought something was wrong, but no.
