After you install this, it will wipe out your data on your Palm!!!!

From the readme:

1. IMPORTANT - READ THIS BEFORE YOU INSTALL ON MAC OS 10.4 "TIGER"

If you are running Mac OS X 10.4 "Tiger" or later and have already been syncing your Palm OS device with this Mac, you must first perform one final sync with your existing setup before installing this version of The Missing Sync for Palm OS to ensure that your Mac contains the most current data. After installing The Missing Sync 5.1, you will be setting the Mark/Space conduits for Contacts, Events, and Tasks to overwrite the handheld data for the first sync./b Complete instructions are provided below in the "Installation Instructions" section of this Read Me.

easy, easy... - macDigital

that's why it's always good to read the big red text in the installer. as the instructions read, if you are using Mac OS X 10.4 make sure you sync before installing...and they recommend you back up your information as well since this is a significant upgrade.

i performed a sync, double-checked it for accuracy, backed it up, and then installed the 5.1 update. no problems here, but then i actually read the instructions as well.

Well, that's why they have installation instructions and large red IMPORTANT headings that tip you off on what to do or not do... rather hard to miss actually. As macDIgital said, follow the installtion directions and all will be good.
