Unfortunately this is the only real option for MIDI communication in RealBASIC. It is good but support is non-existant. This developer completely ignores his customers. He refuses to respond to email and even his own support form on his website. This is a useful program but it is no where near complete. There are things that need to be fixed with this but it is never going to happen under the current support model. If something else was available, I'd use it, but there isn't. I'm hoping that if enough users complain in public forums like this, the developer will finally respond.
