If it's suposed to be "Free", than knock off the requirement of supplying an e-mail address you can sell.

I've tried out a handful of survey services and programs, and this one is a clear winner. Form creation is a breeze, the interface is intuitive, and scripting adds a remarkable degree of flexibility. It's good enough that I set it up on a Windows box and use VNC to access it (I'm a Mac OS X user myself). The only part that I found to be slightly difficult was using my own computer as the server for publishing the survey: the documentation says that php must be enabled (true), but in my case I also had to change the default permissions for newly created files (see umask) and the permissions for the data directory ("other" must have rwx access) as well as activating Postfix/sendmail before my server passed muster. Not much guidance in the docs on these; the assumption is that all servers will be properly configured, which many won't be by default. Other than that, though, no complaints at all. The best thing bar none is the tech support, which is instantaneous, copious, friendly, and helpful above and beyond the call of duty. Now if they'd just come out with a Mac version....
