Basically, what it does is allow you to drag a photo in its window, and then you can position it or resize it (if you want). Then click a button and it basically crops the photo down to the size it should be.
Then drag it into iPhoto to sync when you do your next sync. Note it also uploads the photos via USB and Bluetooth to some phones, but not the iPhone. I'd prefer to do it all from iPhoto anyway.
All in all, it's a good app. There is a Lite version for less money too.
