Product Description:

Formulate is a simple Mac OS X application that allows one to overlay text on top of a PDF file. This is particularly useful for filling out forms. The interface is not the best, but this program is a lot cheaper than Acrobat. There is also a good tutorial on the website to help you get accustomed.

What's new in this version:

German and Spanish localizations, small UI tweaks.

This software was renamed FormulatePro: http://www.versiontracker.com/dyn/moreinfo/macosx/33391

Despite the name, it's still GPL freeware.

I would have loved this if I hadn't discovered FormMate first (www.whitewolf.com). FormMate costs $29.95, and Formulate is free, so that's a big difference . . . but there is also a big difference in the applications. FormMate is considerably easier to use, and the saved documents can be saved just like any other doc or pdf file. You can customize the fonts globally or per document, and can store frequently-used data for repeated use. You can size the window so that if you're old (like me) and need larger type, you can enlarge in a flash. If I only needed to fill in pdf forms once in a while, I'd go with Formulate. But I fill them in several times a week, and FormMate works much better for that amount of use.
